Caregivers
- Have a minimum of 3 years of experience
- Present acquire work history
- Strong Communications skills
- Written Letters of Reference:
- We ask that you provide at least 2-3 written professional references from any of your past clients, or their family members. These must be from people who have observed your work first-hand with clients and who are impartial sources, rather than your personal friends or co-workers.
- Identification and Certification:
- We need you to submit three (3) forms of identification (proof of legal status, Social Security card, government-issued photo ID) as well as any professional credentials you may have (CNA/HHA/PCA, etc.) You may also submit diplomas, resumes or relevant training certificates (CPR, first aid, in-service training).
- A Photograph:
- We ask that you submit a clear professional head & shoulders photograph for us to use when we present you to potential clients.
Private Duty Nurse Requirements
- Valid RN License
